We are looking to recruit a Community Care Officer to our Deaf team to provide services as a member of a multi-disciplinary and dynamic Adult Sensory Needs Assessment and Support Service to Deaf, hearing impaired and Deafblind people. To support the delivery of Bradford’s vision for Community Led Support and our Home First strategy, the successful candidate will be skilled and motivated with excellent interpersonal skills and able to work with individuals as well as groups. The successful candidate will also provide elements of an enablement role in promoting independence with a focus on communication.

The main responsibilities of the post are to work alongside people who are Deaf, hearing impaired and Deafblind, providing assessment and support to enable individuals, and carers where appropriate, to achieve their desired outcomes.   • Contribute to the provision of Social Care Services in the Community and other settings. Assessing and/or evaluating the needs of adults who are Deaf, hearing impaired or Deafblind and carers including young carers, identifying & facilitating appropriate options to meet the desired outcomes of the individual. 
  • Undertaking assessment, support planning and reviewing within Council policies and procedures.  
  • Undertaking DeafBlind assessments as informed by the Care Act 2014
  • Ensuring that statutory provisions are met, and the standard and quality of support is appropriate and meets Department Policies and guidelines.
  • Work closely with partner agencies to support Deaf, hearing impaired and Deafblind people. To link in with health colleagues, enabling access to primary services, enabling people to be more aware of their rights and how to meet health needs. 
  • Assessing and reviewing High-Cost Social Care packages, ensuring services are appropriate, effective and cost efficient 
  • Using the Line Management processes, obtain the advice and agreement of a Line Manager to commit budgets and resources to packages of care negotiating services or resources as necessary, and recommending service developments, ensuring that statutory provisions are met, and the standard and quality of care is satisfactory and meets Department policies and guidelines.
  • It is essential that you have British Sign Language (BSL) skills, either as a native language or have a minimum qualification at level 2.

  • Level 1 Community Care Officer
  • Must have a wide range of life and social experience including voluntary work. Recent experience of paid employment working with adults in either a health or social care setting. Experience of working with people who are Deaf, hearing impaired or deafblind.
  • Level 2 Community Care Officer
  • Experience in a Level 1 post or equivalent paid employment experience working in the assessment and provision of care of adults. Experience of working with people who are Deaf, hearing impaired or deafblind.
  • BSL stage 2 minimum qualification or native BSL user.
  • Level 3 Deafblind assessor qualification.
